Admission Criteria

Patients diagnosed with a life-limiting condition and given a prognosis of six months or less may be eligible for hospice care. Eligibility is confirmed through consultation between an attending physician and our medical team.

Eligibility Guidelines:

• A confirmed diagnosis of a life-limiting illness.
• A prognosis of six months or less if the condition runs its natural course, certified by an attending physician.
• A preference for a palliative plan of care.
• A request from the patient and family to receive services from DIVINE LIGHT HOSPICE LLC.

Referral Process:

Most referrals are initiated by a primary care physician, but they can also come from health professionals, family members, friends, assisted living communities, or the patients themselves. Please reach out directly to request an evaluation and supportive guidance.
